First Visit

We pride ourselves in providing comprehensive care to our community. Your initial appointment usually consists of a cleaning, only the necessary diagnostic x-rays, and a comprehensive examination, wherein we evaluate, diagnose, and review your treatment options. While we would like to complete treatment as soon as possible, please keep in mind that a complex medical history or treatment plan may require further evaluation and a second appointment.

We will also see folks for urgent and emergency care. At this type of visit, we will complete a problem focused exam and take only the x-ray(s) we need to address the immediate concern(s).

Please assist us by providing the following information at the time of your appointment:

  • Any x-rays taken by a previous dentist, if applicable.
  • A list of medications you are presently taking.
  • All of the information necessary to submit your dental insurance claim. This information is: the dental insurance company, subscriber/member ID, group ID, and the name, date of birth, and mailing address of the subscriber.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian. 

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your previous provider has taken x-rays, please request that they forward them to our office. If additional x-rays are necessary, they will be taken at our facility.
