Dental insurance plans we work with:

  • We are In Network with Anthem Blue Cross Blue Shield: Dental Complete plans and Grid+ plans.
  • We are considered Out of Network with all other insurance plans. As a courtesy, we will still submit the claim for you (with the exception of Medicaid, SunLife, and AFLAC) and your plan will likely pay at least a portion of your services. However, you are still responsible for all estimated copays and any remaining balance that may be due even after your insurance claim processes. Please discuss your Out of Network benefits with your insurance plan prior to scheduling.
  • IMPORTANT: Effective June 1st, 2025, all providers at Gunnison Family Dentistry are Out of Network with all Delta Dental plans. As stated above, we will still submit the claim to Delta Dental for you as a courtesy. However, Delta Dental will not pay to Out of Network providers and, instead, will pay the policy holder if the plan has Out of Network coverage. Patients with Delta Dental who elect to be seen at Gunnison Family Dentistry are required to pay in full at the completion of every appointment.

The Gunnison Family Dentistry Membership Plan:

This is a membership plan for those who do not have dental insurance. This 12-month plan covers your 2 routine Hygiene visits (including routine cleanings, exams, and x-rays), a problem-focused exam, and provides a 10% discount off of all of your treatment needs within our office.

Payment is due at the time of service.

As a courtesy to you, we will bill your insurance company*. Please keep us informed of any changes to your insurance plan. Patients with insurance are required to pay all estimated co-pays at the completion of their appointment.

*Medicaid Policy: We ARE NOT a Colorado Medicaid-approved provider. People on Medicaid must see a Medicaid-approved provider. Anyone who attempts to obtain services without disclosing their Medicaid status will be reported to Medicaid for demonstrating an ability to pay and may lose their Medicaid benefit. The person will also be dismissed from our practice. We take fraud cases seriously in our practice and will not tolerate any attempts to do so.
